Every child that attends Kids On Track needs a registration form for
that program. If your child would like to invite a friend to a program,
please download an extra form and have their parent sign the form
before they attend. You need to only fill out one registration form per
child for any of the programs. PLEASE NOTE: Only a legal
guardian of the child can sign a registration form.
